This site includes links to all of the surveys for the Homework Connection program. The surveys can be accessed in the table below. They can be printed and given directly to clients or be completed online.

| Tutors: |
- Tutor - Before– is completed
by the student’s tutor at the start of the
student’s work with the Homework Connection.
- Tutor - After – is completed by
the student’s tutor after the student has
completed their work with the Homework Connection.

- Student - Before – is completed by the student
as they first enter the program.
- Student - After – is completed by the
student when the Homework Connection program is completed.
