Project Connect Surveys –
Connect Counselors
This site includes links to all of the surveys for the Connect Counselor
program. The surveys can be accessed in the table below. They can be printed and given directly to clients or
be completed online.
The Connect Counselor is responsible for distributing and collecting
the surveys to themselves and the students.
Besides the Connect Counselor, each school corporation also
has an Evaluation Contact person who is responsible for distributing
and collecting the surveys for teachers and guidance counselors.
The contact person will also gather the completed surveys from
the Connect Counselor and send them to Strategic Development
Group for compilation.
